Medication Awareness

Medication is a critical part of many clients' care. Whether or not you are trained to administer medication, you must understand what to observe, when to escalate concerns, and how to keep clients safe. Never administer medication unless you have been specifically trained and signed off to do so.

The 6 Rights of Medication

  • Right Person — Confirm the identity of the client before any medication is given
  • Right Medication — Check the medication matches the prescription and the MAR chart
  • Right Dose — Verify the correct dose as prescribed. Never alter a dose without authorisation
  • Right Time — Administer at the time specified in the care plan and prescription
  • Right Route — Confirm the correct method of administration (oral, topical, inhaled, PEG, etc.)
  • Right Documentation — Record the administration immediately, accurately, and completely

What to Observe and Report

Never Administer Without Training

You must never administer medication unless you have completed the required training and been formally signed off as competent. This applies to all routes of administration. If you are asked to give medication you have not been trained for, refuse and contact your manager immediately.

Reporting Concerns

If you have any concerns about a client's medication — whether it relates to side effects, missed doses, storage, or errors — report it to your manager or the clinical team immediately. Do not wait until the end of your shift. Prompt reporting can prevent harm and ensure the client receives the right care.
